Would you like to work in an outreach capacity to support our most vulnerable residents into treatment. Greenwich has a dynamic and experienced team who would love to hear from you.
The Role
We are looking for a dynamic and driven individual who is committed to providing assertive outreach to those who use our services. Could you help engage or re-engage the homeless population into wellbeing services and drug and alcohol treatment. You will be able to work both independently and as part of a supportive team to:
- to carry out triage and comprehensive substance misuse assessments, risk assessments and develop person-centred Recovery Care Plans predominantly with them.
- to conduct street outreach to engage/re-engage homeless individuals misusing substances.
- to conduct assessments at local hospitals and liaise with their substance misuse team on a daily basis.
- to develop strong links and partnerships with a wide range of community groups aimed towards vulnerable people to encourage and promote access into services for people.
- to deliver satellite provisions within local services e.g., GPs, probation, hostels etc.
- to deliver harm reduction advice, well-being, education, and support.
- to work in collaboration with partner agencies in housing, immigration, healthcare, etc.
The Service
Via Greenwich is a fully integrated substance misuse service, providing clinical and psychosocial interventions, operating from a main treatment hub in Woolwich. The team actively promotes recovery and works with individuals that are directly and indirectly affected by substance misuse, working with the local community to improve the health, wellbeing and functioning of Greenwich residents, irrespective of circumstance.
Via Greenwich Navigation team aim to improve service integration, as well as engaging and connecting with partners across the borough to create sustainable pathways. The team will ensure wrap around support for complex people including those in the Criminal Justice system.
